The US Food and Drug Administration (FDA) inspected Dabur India Limited’s drug manufacturing facility at Survey No. 225/4/1, Village Saily, Silvassa, Dadra and Nagar Haveli and Daman and Diu (FEI 3001413302) from 12 to 16 January 2026. The inspection resulted in Warning Letter 320-26-105, issued on 24 July 2026 and published on 4 August, citing significant violations of Current Good Manufacturing Practice (CGMP) regulations under 21 CFR Parts 210 and 211.

FDA concluded that the over-the-counter (OTC) drug products manufactured at the site are adulterated under section 501(a)(2)(B) of the Federal Food, Drug, and Cosmetic Act, on the basis of failures in quality unit oversight, laboratory record completeness, cleaning validation and process validation.

The firm’s 4 February 2026 response to the Form FDA 483 was found inadequate against every citation. In each case, the reason given by FDA was the same: the corrective actions treated the observations as isolated procedural gaps rather than as evidence of a systemic breakdown in quality oversight, and the retrospective assessments proposed were too limited to establish the impact on product already distributed in the United States.

The enforcement position had already escalated well before the letter was issued. FDA placed all drugs offered for import from the firm on Import Alert 66-40 on 5 June 2026, following a voluntary recall issued on 2 June. The letter records that certain drug products had still not been recalled at the time of writing.

Key Non-Conformances Identified

FDA cited four CGMP violations. They are not independent findings. The data integrity failures under citations 1 and 2 removed the evidence base that would normally support product impact assessment, and the validation gaps under citations 3 and 4 meant there was no process knowledge to fall back on once the records were in question.

Non-Conformance 1: Failure to Establish an Adequate Quality Control Unit

Regulatory Citation: 21 CFR 211.22(a)

FDA describes a quality unit that did not exercise its authority over manufacturing operations, including over the integrity of production records.

Key Findings

  • Falsified equipment usage logbook:
    • Requested for a line that management stated was dedicated to a single product
    • Production of the record was delayed by several days
    • The version provided appeared to have been newly created
    • The original was subsequently located by the investigator in the document room
    • The version provided omitted multiple US marketed OTC drug products documented in the original
  • Discrepancies between source data and reported values:
    • Analytical data books did not match the values in batch manufacturing records and certificates of analysis
    • Affected attributes included residue on ignition, melting range, pH, and viscosity
    • No out-of-specification results were involved
    • The quality unit signed off on the records without verifying consistency and accuracy

FDA’s Core Concern

The absence of an out-of-specification result is not a defence. FDA cited the transcription discrepancies alongside the falsified logbook because both demonstrate that the quality unit was approving records it had not verified. 

The firm attributed the logbook to inadequate documentation controls, insufficient oversight, and a lack of robust governance, and attributed the remaining discrepancies to transcription practices and insufficient review resources. FDA found the response inadequate because it did not address the quality unit’s failure to ensure the integrity of all data, did not assess the potential impact on product already distributed, and did not evaluate whether the quality unit is capable of fulfilling its required functions at all.

Non-Conformance 2: Failure to Ensure Complete Laboratory Records

Regulatory Citation: 21 CFR 211.194(a)

Laboratory records did not include the complete data derived from all tests necessary to demonstrate compliance with established specifications.

Key Findings

  • Microbiological plates:
    • Recorded in analytical notebooks as incubated, for both finished drug products and media samples
    • Physically absent from the incubators
    •  No documented explanation of their whereabouts or disposal
  •  Missing analytical documentation:
    • Test procedures, analytical workbooks and test data sheets could not be produced for a number of batches of US marketed drug products
    • Certificate of analysis results were therefore unsupported at the time of inspection
    • Equipment logbook entries were not made for instruments used in batch release testing
  • Firm response and FDA assessment:
    • Missing plates attributed to a microbiologist discarding them without training
    • Retraining, re-analysis and a new controlled logbook proposed
    • Retrospective testing offered for several affected batches only
    • FDA found no comprehensive retrospective risk assessment and no complete testing of all potentially affected product within expiry

FDA’s Core Concern

FDA states plainly that data reliability is fundamentally compromised where records of test results are not made or not maintained, and that the absence of reliable data removes the ability of the quality unit to perform its function. 

The remediation requested goes well beyond retraining. FDA asks for an investigation protocol and methodology, interviews with current and former employees conducted by a qualified third party, identification of omissions, alterations, deletions, record destruction and non-contemporaneous completion, a statement of whether the individuals responsible remain able to influence CGMP data, and a commitment to annual audits by a qualified consultant for at least two years. The letter also asks the firm to confirm whether it will appoint a chief integrity officer empowered to receive anonymous employee complaints.

Non-Conformance 3: Failure to Establish and Follow Adequate Cleaning Procedures

Regulatory Citation: 21 CFR 211.67(b)

The firm did not demonstrate that its cleaning practices remove potential contaminants or prevent carryover on shared equipment used for OTC drug products.

Key Findings

  • Cleaning validation on a non-dedicated line relied exclusively on visual inspection
  • The programme lacked:
    • Scientifically determined maximum allowable carryover limits
    • Quantitative residue testing
    • Direct surface sampling of difficult-to-clean areas
  • A second line had no cleaning validation at all, despite being used for several different OTC drug products
  • Firm response and FDA assessment:
    • Validated analytical methods, carryover limits, swab sampling and revalidation committed to
    • One line stated to be newly dedicated to a single product
    • Retrospective review limited to a small number of samples, which FDA describes as statistically insufficient
    • No interim controls proposed to manage cross-contamination risk during remediation
    • No retrospective review of all US distributed product within expiry

FDA’s Core Concern

Without carryover limits derived from toxicological data, a validated analytical method capable of detecting residue at those limits, and sampling of the locations hardest to clean, there is no evidence that the equipment is clean rather than merely visually clean. 

FDA sets out the worst case parameters it expects the revised program to address, including higher toxicity and higher potency products, products with low solubility in the cleaning solvent, products that are inherently difficult to clean, worst case swab locations and maximum dirty hold times.

Non-Conformance 4: Failure to Establish Adequate Production and Process Control Procedures

Regulatory Citation: 21 CFR 211.100(a)

Key Findings

  • Process validation had not been performed for most of the drug products manufactured on two of the firm’s units
  • The firm acknowledged the gap and committed to validation plans and protocols on a campaign-based approach
  • The response contained no schedule or timeframes for completing validation product by product
  • No interim plan, such as enhanced finished product testing, was proposed for product manufactured and distributed before validation is complete
  • No product impact assessment was provided for US-distributed product within expiry or for ongoing manufacture

FDA’s Core Concern

FDA restates the lifecycle position: process qualification studies must be completed before commercial distribution, and ongoing monitoring of intra-batch and inter-batch variation is required thereafter to demonstrate a continuing state of control. A commitment to validate in the future does not address product already on the market, which is why the agency asks for the interim testing plan and the impact assessment alongside the validation timeline.

CAPA Expectations and Regulatory Recommendations

FDA recommends that the firm engage a qualified consultant to perform a comprehensive six-system audit of the entire operation and to evaluate the completion and effectiveness of corrective actions before pursuing compliance status. 

The letter is explicit that using a consultant does not relieve the firm of its own obligations, and that executive management remains responsible for resolving the systemic flaws. The expectations below are drawn from the remediation requested in the letter.

Data Integrity and Quality Unit Oversight

Root Causes

  • Quality unit without the authority or resources to challenge production records
  • Documentation controls that permitted parallel and reconstructed records
  • Review practices that confirmed the presence of a signature rather than the accuracy of the data
  • No mechanism for employees to raise data integrity concerns confidentially
  • Remediation scoped to the observation rather than to the system

Expected Corrective and Preventive Actions

  • Comprehensive assessment of documentation systems across manufacturing and laboratory operations, identifying where practices are insufficient
  • CAPA plan ensuring records are attributable, legible, contemporaneous, original and accurate throughout the operation
  • Independent investigation into the extent of data inaccuracies, covering:
    • A documented protocol and methodology, with justification for any operation excluded from scope
    • Third party interviews with current and former employees to establish nature, scope and root cause
    • Identification of omissions, alterations, deletions, record destruction and non-contemporaneous completion
  • Current risk assessment of the effect on drug quality, including risk to patients from product already released
  • Assessment of quality unit authority, resourcing, procedural robustness and batch review practice
  • Confirmation of whether individuals responsible remain able to influence CGMP data
  • Qualified consultant audits at least annually for two years to verify CAPA effectiveness

Laboratory Data and Sample Control

Expected Corrective and Preventive Actions

  • Complete retrospective testing of all potentially affected product within expiry, not a sample of it
  • Reconstruction or reconciliation of certificate of analysis results against available raw data, batch by batch
  • Controlled traceability for microbiological samples from preparation through incubation to disposal
  • Instrument use logs reconciled to batch release testing, with the reconciliation itself subject to quality review
  • Quality assurance oversight of laboratory data review, distinct from the laboratory’s own second check

Cleaning and Cross-Contamination Control

Expected Corrective and Preventive Actions

  • Independent retrospective assessment of cleaning effectiveness across every piece of equipment used for more than one product, identifying residues and whether cross-contaminated product may have been released
  • Carryover limits established on a scientific basis, with validated analytical methods capable of measuring against them
  • Direct surface sampling at worst case locations, replacing visual inspection as the sole acceptance criterion
  • Worst case evaluation covering toxicity, potency, solubility, cleanability and maximum dirty hold time
  • Interim controls for the remediation period, defined and documented rather than implied
  • Change management steps required before new equipment or a new product enters the facility

Process validation

Expected Corrective and Preventive Actions

  • Validation program covering the full product lifecycle, with associated procedures
  • Product by product timeline for process performance qualification
  • Process performance protocols and written procedures for equipment and facility qualification
  • Monitoring of intra-batch and inter-batch variation to demonstrate an ongoing state of control
  • Interim enhanced testing for any product manufactured before qualification is complete

Timeline for Implementation

Immediate Actions (0 to 3 months)

  • Respond to FDA within 15 working days, or state the reason for delay and a completion schedule
  • Engage an independent cGMP consultant and agree the scope of the six system audit
  • Secure and sequester all original records, and stop any practice that permits parallel record keeping
  • Extend the recall assessment to the product families not yet covered
  • Implement interim controls for cross-contamination on non-dedicated lines

Short-Term Actions (3 to 6 months)

  • Complete the retrospective data integrity investigation, including third-party interviews
  • Complete retrospective testing of affected product within expiry
  • Issue revised procedures for documentation practice, data review, cleaning validation and batch release
  • Establish carryover limits and validated residue methods, and begin cleaning revalidation
  • Publish the product-by-product process validation schedule

Long-Term Actions (6 to 12 months)

  • Complete process performance qualification against the published schedule
  • Demonstrate CAPA effectiveness with data rather than with completed training records
  • Embed management review of data integrity metrics and investigation quality
  • Operate the confidential reporting route and demonstrate that concerns raised are investigated
  • Prepare for re-inspection, which is the route to resolving the import alert

Conclusion

This warning letter is a case where a logbook was reconstructed, analytical values were transcribed into certificates of analysis without verification, plates recorded as incubated could not be found, and batches were released without the supporting analytical records existing at all. In each instance, the quality unit approved the outcome.

The pattern in FDA’s assessment of the response is the part worth carrying into your own site. Every proposed corrective action was rejected for the same reason: the firm scoped its remediation to the observation. Retraining the microbiologist, dedicating the line, testing a few samples, and revising a procedure are all reasonable actions, and none of them answers the question the agency is actually asking, which is what else is affected and how would you know.

Where records are unreliable, there is no evidence base for a product impact assessment, and where processes are unvalidated, there is no process knowledge to substitute for it. That combination is why the escalation ran from inspection to import alert in under five months.
